1. MSD assay plate barcode
2. MSD assay plate lot number (if available)
3. MSD Sales Order number (if available)
4. Instrument serial number (on the back of the instrument)
5. Instrument software name:

• Methodical Mind, OR
• DISCOVERY WORKBENCH

6. Instrument software version
7. How you log in to the Methodical Mind software:

• Methodical Mind login credentials (email address and password), OR
• Windows username and password

8. Your contact information, including:

• Company name and location
• Email address
• Phone number
